NYC DOE Health Screening

Students are required to fill out a survey each day before entering the school building. The survey can be filled out online or a paper will be provided when you arrive at school. Students filling out the paper form at school will undergo a temperature check. There will also be random temperature checks for students who complete the survey online.

How To Complete Daily Health Screening:

  1. Go to

  2. Click on the Guest Screening Button for Students, Families and Visitors.

  3. Click “I am a visitor or family member” and enter your details.

  4. In the school field, type our official name Baruch College Campus High School, and our school will pop-up from the drop-down list.

  5. Answer the 4 questions and hit submit.

  6. You will receive a Cleared to Enter DOE Schools and Facilities Message such as the one below. We ask that this documentation is presented whenever you enter the building.

  7. If a caregiver does not have proof of the completed online health screening, s/he will be asked to complete a paper version of the questionnaire provided by one of our safety agents before entering the building.

In order to keep everyone healthy and slow the spread of COVID-19, students will not be able to attend in-person classes if they have:
  • Experienced any symptoms of COVID-19, including a fever of 100.0 degrees F or greater, a new cough, new loss of taste or smell, or shortness of breath within the past 10 days;
  • Received a positive result from a COVID-19 test that tested saliva or used a nose or throat swab (not a blood test) in the past 10 days;

  • Been in close contact (within 6 feet for at least 10 minutes) with anyone who tested positive for COVID-19 or who has or had symptoms of COVID-19 in the past 14 days; or

  • Traveled internationally or from a state with widespread community transmission of COVID-19 per the New York State Travel Advisory in the past 14 days.

Students who fail the health screening should contact their school to notify the school of their absence and to discuss how to remain engaged in remote learning.

Where To Get Tested For COVID-19

As stated in the Chancellor’s letter of September 21st, it’s up to all of us to keep our school communities and families safe by wearing face coverings, keeping our distance, washing hands frequently, and getting tested!

  • You can learn more about the virus, its symptoms, and how it spreads here. Please note that a positive test result requires a 14-day quarantine period.
