Registration
If your child has received an offer to attend Baruch College Campus High School, please read through the information on this page to begin the registration process.
The following forms must be completed by all incoming students:
- Online Blue Card & Parent Consent Form
- Health Form: This form must be filled out by your healthcare provider and can be returned in the fall
- Review the Immunization Requirements Family Letter and submit a complete immunization record for the student
The following required forms are for students who are new to the NYC Department of Education, coming from private schools, charter schools, out of state, etc:
- Housing Questionnaire
- Home Language Identification Survey
- Parent-Guardian-Student Ethnic & Race Identification Form
All forms are fillable pdf files and can be completed electronically and emailed to records@bcchsnyc.net.
